At least one direct ancestor (parent, grandparent, or great-grandparent) who was a Czechoslovak or Slovak citizen born on the territory of today's Slovak Republic.
Documented direct lineage from the qualifying ancestor to you using full birth and marriage certificates for each generation.
Proof that the qualifying ancestor held Czechoslovak/Slovak citizenship (for example, passport, citizenship certificate, military booklet or registry extract).
Evidence that the ancestor did not lose Czechoslovak/Slovak citizenship before the birth of the next generation in your line (foreign naturalization and immigration records).
Clean criminal record and good character; no serious tax, security or immigration violations.
Willingness to obtain Slovak permanent residence as part of the citizenship-by-descent process; no Slovak language or long-term residency requirement for this route.
